Overview

Strive Health

Strive Health is built for purpose- to transform a broken kidney care system. We are fundamentally changing the lives of kidney disease patients through early identification and engagement, comprehensive coordinated care, and home-first dialysis. Strive’s model is driven by a high-touch care team that integrates with local providers and spans the entire care journey from CKD through ESRD, leveraging comparative and predictive data and analytics to identify patients at risk. Strive Health’s interventions significantly reduce the rate of emergent dialysis crash, cut inpatient utilization, and significantly improve patient outcomes and experience. Come join our journey as we create THE destination for top talent in the Healthcare community and set a new standard for how care kidney should be done.

EPI Dialysis Program Manager

The Enterprise Program Innovation (EPI) function maintains visibility across the entire landscape of our enterprise, understands Strive’s overall strategy and OKR’s and helps to align and integrate each service line to reach OKR’s and other deliverables.

The Dialysis Program Manager (DPM) oversees the dialysis program, designing and coordinating innovation projects.  It is because of the opportunity to assist with design and creation of projects for the dialysis program, that this role is distinguished from other Program Manager positions.

The combination of EPI & DPM works to integrate the dialysis program with all other programs across the enterprise.  This role reports to our Senior EPI-PM, with a dotted line to dialysis program leadership dyad: our senior medical director of dialysis and senior VP of dialysis operations.
